Adult REVIVE

(Ages 14 and up)

$450/year

  • 2 maintenance dental cleanings** per year
  • 2 doctor exams per year
  • 2 fluoride varnish treatments per year
  • Annual check-up x-rays
  • 15% off other treatments, including Invisalign
    • **Initial perio therapy excluded but Perio Maintenance included

Child REVIVE

(Up to age 13)

$400/year

  • 2 maintenance dental cleanings per year
  • 2 doctor exams per year
  • 2 fluoride treatments per year
  • Annual check-up x-rays
  • 15% off other treatments, including Kids + Teen Invisalign

Family REVIVE

 

$1500/year

  • Includes 2 Adults + 2 Children in same household
  • $400 surcharge for each additional family member
  • Same preventative benefits apply to each patient
  • Same 15% off other treatments, including Kids + Teen Invisallign
